Angeles Southwest College Fixed Asset Change Request Form
This form must be completed and forwarded for approval prior to moving any inventoried asset on campus. With the new barcode system all assets valued at $500 and above are being tracked. Monthly audits will be conducted by the District Office and College staff to make sure that equipment is in the correct location. Please submit this form on-line indicating the name of the approving supervisor. The form will be received by the Vice President for Administrative Services for review and for final approval. DO NOT MOVE EQUIPMENT UNTIL YOU ARE NOTIFIED OF APPROVAL.
